What is a hazard?
A hazard is any source or situation with the potential to harm humans, potentially causing physical or psychological injury, ill-health, or a combination thereof. If you encounter a situation that could negatively impact a worker's health and safety, it should be reported immediately.
Optus has developed a Public Hazard Form for reporting hazards, streamlining the process for both the public and Suppliers Key Users (CKU). This form allows for the advising and reporting of hazards without the need to access DoneSafe.
