Step 1: To be completed by an authorised representative of the deceased

Name of the deceased account holder

Authorised representative's details

You are either an executor/administrator of the estate or can produce a statutory declaration if required. We may need to get in touch with you if clarification is required.

Step 2: Optus service/account number to be updated

Please list each Optus service/account number or username that requires updating. Please indicate below if the service(s) should be transferred to a new account holder or cancelled

Only required if transferring ownership of any of the above services
