NSW: A COVID-19 digital certificate displayed through the Express Plus Medicare app, Service NSW app or equivalent smartphone app; a printed version of the COVID-19 digital certificate or an immunisation history statement; a Service NSW QR code check-in for the store that shows your vaccination status; or a NSW Health contraindication form issued by a registered medical practitioner or a Medical Clearance Note issued by NSW Health.

Metro Melbourne: A COVID-19 digital certificate displayed through the Express Plus Medicare app, Service Victoria app or equivalent smartphone app; a printed version of the COVID-19 digital certificate or an immunisation history statement; a Service Victoria QR code check-in for the store that shows your vaccination status; or a medical exemption from a CHO-approved medical practitioner
