What do I do if my phone is lost or stolen?
If your phone is lost or stolen, you should do the following:
1 Report it to Optus (1300 133 334 or +61 2 8082 5678 if calling from overseas)
As soon as you notify us, we will temporarily bar or suspend your service to prevent unauthorised use of your SIM Card. You will also be asked if you would like to block the handset's IMEI number. This will prevent unauthorised use of the handset. If you do not restrict your service, you will be liable for any unauthorised calls.
2 Report it to the Police
Give the Police full details of when and where it was lost or stolen, plus the phone make and model number along with serial number (also referred to as IMEI or ESN number). You will be asked for details of this Police Report when you make a claim.
3 Proceed with Optus Mobile Insurance claim (if covered)

What do I do if my phone is accidentally damaged?
If your phone is accidentally damaged, you should do the following:
1 Report it to Optus
Call 1300 133 334 or +61 2 8082 5678 if calling from overseas.
2 Obtain a Damage Report
Visit an Optus World Store who will send your phone to an Optus Mobile Service Centre for assessment.
3 Proceed with Insurance claim (if covered)
